Ear wax are normal secretions to protect the ear canal, if the patient tries to clean them by cotton tip applicators, they will be more stuck and pushed deeply towards the ear drum, then they will cause hearing loss which is reversible by cleaning at the doctor.
Nothing should be allowed to go into the ear canal! (Cotton tips, keys, pen covers, fingers....)
As for smoking, it is not related to wax, but might cause hearing impairment on the long run by blocking the very small arterioles that supply blood to the hearing center.
