My 3 months old baby has been very irritated in the past 2 days, water came out of his ear (showered 2 days ago) and no fever. Should I be concerned

It could be that water was inadvertently into his ear while he was being bathed and now got out, but we need to make sure that there is not redness or residual irritation of the ear canal as a result of the water having been stuck inside for 2 days, so I strongly recommend that you have your baby examined by his pediatrician to avoid complications like otitis externa (infection of the ear canal).
