If it's the prostate that is causing the obstruction, a medical therapy could help you a lot.
Otherwise, if there is a stricture more distally in the urethra, a dilatation must be performed if deemed necessary.
I advise you to consult a urologist who could help you evaluating and treating your condition.

You may have to treat for emphysema first and then urethral dilatation.
