Some patients may have normally one testis bigger than the other,but this difference in size may be due to hydrocele (water around the testis) or inflammation of testis or if there is induration you need to eliminate other serious problems

May I add another comment to the valid notes of the esteemed Dr Khalil that some patients perceive the natural more descent of the left testicle as being larger than the right one.
