- yes it is possible, although rare,you can have a hymen that was a bit lacerated from the first time, than the extent of laceration is enlarged in the second or third time...

- you need to be examined, to see if there is really hymenal laceration, a lot of female will do fingering without breaking the hymen, as it depends on the hymenal architecture & form...
