My 1-year boy had high fever 40+. Infection shown in blood test. Urine test clean. Physical exam clean. Augmentin 5 doses and fever still 39. In crawling wld lift one knee and push with his foot.

The first thing to be rulled out is septic arthritis which is infection at the level of the joint and this needs intravenous antiobiotics to be given and bed rest.
The other thing that we could think about is transient synovitis which is inflammation at the level of his hip or knee which occurs after a viral infection and could be associated with fever (usually not high grade fever). The treatment here is bed rest (as much as possible) and anti-inflammatory medications.
To differentiate between those two entities analysis of liquid from the joint that is affected is required (this is done by an orthopedic surgeon). Before doing this analysis an imaging is required (ultrasound or Xray). Blood test and culture is also required.
Other causes could be also responsible it all depends on the physical exam done by his physician.
